Revive’s hormone evaluation and ongoing hormone programs are self-pay and are not billed directly to insurance.
We accept HSA and FSA funds, as well as CareCredit, for eligible services. Some prescriptions may also be sent to an outside retail pharmacy. For example, estrogen patches are not included in the monthly program and may be eligible for insurance coverage depending on your individual plan and pharmacy benefits.
We’ll clearly explain what’s included in your Revive program and any potential outside pharmacy costs before you begin treatment.
